Neive is the most powerful and tannic of Barbaresco's three communes, producing wines of extraordinary concentration and longevity from its mix of Helvetian and Tortonian soils in the Tanaro valley's northern hills. The commune is anchored by Bruno Giacosa's legendary Santo Stefano — a south-facing amphitheatre of vines whose Riserva, in exceptional years marked with a red label, is among the most sought Italian wines at auction anywhere in the world. Santo Stefano, Gallina, Albesani, Serraboella, and Basarin are the most celebrated MGAs, each producing wines with the power and age-worthiness that distinguish Neive from the more immediately approachable Barbaresco commune. Castello di Neive has been the most consistent exponent of Neive's traditional style across all five MGAs; Paitin and Ca' del Baio offer more modern, polished interpretations of the Gallina and Serraboella sites.

Neive Barbaresco is the most demanding and powerful expression of the DOCG: firm, structured tannins, high natural acidity, and a concentrated core of dark cherry, iron, and dried herbs that requires substantial patience. Giacosa's Santo Stefano Riserva is almost impenetrable in youth — tar, iron, and tightly coiled fruit — but evolves over 20–30 years into a wine of extraordinary complexity. Castello di Neive's wines are more immediately accessible; Paitin's Gallina offers a modern polish while retaining Neive's characteristic structural grip.
